Poor Old Noble Dog…. (Update 2012-11-13)

This post has nothing to do with homebrewing…. Sorry. Just wanted to let everyone know that one of the Two Noble Dogs is going through some tough times right now. Last Sunday Pica (short for Picasso) was quite lethargic and not eating her food. She has also lost a bunch of weight recently. We didn’t think too much of the weight loss because she was getting a lot more exercise playing with the new puppies next door.


When we got to the vet we found out the ol’ girl was suffering from kidney failure. She is almost 12 years old so we were thinking the worst. They ran test after test and never came back with good news. We were faced with that awful decision that I’m sure many of you have gone through in the past. That being “how far do we take this?”. Do we accept what they are telling us or do we fight to keep her going and hope that it is not fatal. Well….We rolled the dice and gave her a shot. You gotta give ‘em a shot right? Right???


So, we settle in for the long haul. We were all in at this point and make no mistake, this was primarily because of my wife’s maternal instincts.


Pica settled in for her stay (much to her chagrin). She was poked and prodded,  petted and loved by the great staff at the vet hospital for almost an entire week. And, although she was treated like a princess and made some new friends, she was very happy to finally get to go home.


As it turns out, the kidney failure was caused by “something” and not just a condition of old age. Meaning, she got into something bad which caused things to start heading south. Luckily, we caught it in time (or so we hope) and began flushing her system in time to minimize the permanent kidney damage. As it stands now, we have her on a battery of meds and have to give her fluids (via subcutaneous IV) daily. Her appetite improved the day we brought her home, but has fallen off in the last couple of days.  With a countertop full of every variety of dog food, and a refrigerator full of opened and barely eaten cans, we were beginning to get desperate as she turned up her nose at each one.   But we finally found her weakness, a trip to McD’s was in order, she scarfed down 2 plain McD’s hamburgers :-) . And yes it was ok’d by the vet.


Today, I took her back to the vet to get her blood work checked again. Her stamina is down but she is starting to act like her old self. We haven’t gotten the results of her latest blood work back yet, but, we are keeping are fingers crossed.


Please send positive vibes and prayers in Pica’s direction. It can’t hurt. Thanks to all of you.

Update (2012-11-13):


I am writing this with the heaviest of hearts. Pica fought like a champ but in the end it was just too much. We lost her yesterday and the void she has left in our hearts is unimaginable.


We were rescued by her 11 years ago when she was the only puppy at the Humane Society not barking. She was the ugly duckling of the group with ears that were so out of proportion with the rest of her body that we had to name her “Picasso”. That quickly got shortened to Pica but she never really grew into those ears. She chased us on the mountain bikes, hiked with us on the trails, and loved to lay in the creeks. She could destroy a tennis ball in minutes and had an uncanny knack for extracting the squeaker from her toys with surgical precision. She guarded the perimeter and made sure the mailman didn’t linger on the porch for very long. She protected her momma with a vengeance and was truly her momma’s puppy all the way to the end. We’re gonna miss you ol’ girl! Now go find Tanner! He’ll show ya the ropes.

Animal Update: Raisins and Grapes - CBS San Francisco

(KCBS) – KCBS’ Jeff Bell talks to Dr. Jack Aldridge, Director of Veterinary Services at The San Francisco SPCA about the surprising effects of raisins and grapes on dogs.


Raisins and grapes are toxic for dogs to eat and can even cause death.   It’s still not understood why raisins and grapes are poisonous  to some dogs.


About 15 years ago, a computerized system that tracks animal-related medical conditions and toxic substance exposure began noting the association between grape and raisin ingestion and the development of acute kidney failure in dogs.



Symptoms of Raisin and Grape Toxicity



Typically, dogs will vomit within a few hours of eating raisins or grapes.   And the partially digested fruit can often be seen in the vomit, or stool, or both.   Some dogs stop eating and develop diarrhea.   They often become quiet and lethargic and can show signs of abdominal pain.   Chemical changes in the blood occur from 24 hours to several days after the dogs eat the fruit.    These changes in blood chemistry signal damage to the kidneys.   Urine production goes down, and this can result in death.


Dogs can recover from this potentially fatal condition, but quick action is required.  If the dog has just eaten the fruit, vomiting should be induced.     Medication to prevent absorption of potential toxins is also important.   Dogs should be hospitalized and given intravenous fluids for at least 48 hours.    Your veterinarian will monitor blood chemistry daily for at least three days to ensure that kidney failure does not occur.   If there is evidence of renal failure further treatment will be necessary.

Azodyl for Kidney Failure in Cats: An Update on the Evidence

In a recent article about pet supplements for the Science-Based Medicine Blog, I reviewed the dietary supplement Azodyl, marketed for kidney failure in dogs and cats. At the time, the evidence I was able to find was extremely limited, poorly controlled, and subject to a high risk of bias due to association of the research with the company marketing the products. My conclusion was that the theory behind the product was weakly plausible and the evidence insufficient to justify a firm conclusion about efficacy.


An abstract is being presented at the upcoming American College of Veterinary Internal Medicine Forum which reports the results of a controlled study on the use of this product in 10 cats with  chronic kidney disease.

Azodyl Fails to Reduce Azotemia in Cats with Chronic Kidney Disease (CKD) When Sprinkled Onto Food 


The study examined whether there was any difference between commonly measured blood markers of kidney disease, blood urea nitrogen (BUN) and creatinine, in cats given Azodyl and cats given a placebo. The Azodyl was sprinkled on the food since this is commonly how the product is used (making cats take capsules is often difficult). The study was randomized, double blinded, and placebo controlled. The criteria for confirming a positive effect were quite generous, however no difference was found between cats given Azodyl and cats given the palcebo.


The authors concluded:



Based on these results, Azodyl, applied by sprinkling onto food fails to reduce [BUN and creatinin] in cats with [chronic kidney disease]. Whether intact capsule administration reduces reduces azotemia in cats with [chronic kidney disease] remains unknown.



Though generally well-designed, this study was small, and of course single studies are almost never sufficient to provide the final word on a particular therapy. The issue of potential bias for or against a hypothesis is always hard to evaluate objectively, but I am unaware of any direct funding or other involvement of the company in this study. I do know that one of the authors, Dr. Susan Wynn, is a prominent researcher and advocate in the area of herbal and some other alternative therapies, so she certainly would not be expected to have a bias against the product. And negative findings in clinical research are inherently more reliable than positive findings because our studies and our psychology are designed to confirm our beliefs rather than refute them. So while the case is by no means closed, the balance of the very limited evidence is currently against any significant clinical value for this product.
